Climbing and Cope Forms

In order to participate in Project C.O.P.E., Climbing and Rappelling activites in the Council, participants must complete a Personal Health and Medical  Record - Class 1 or a non Scout Medical information form.  Please fee free to print this forms and distribute to your members prior to the climbing event.

Participants are also required to complete a Parental Informed Consent and Hold-harmless and Release Agreement.
