Some points on how to best fill out a scout and leader application:
New Scout and Leader Applications!

The BSA has produced a new application that can be scanned in stead of having to enter the information by hand. There is only
one application for youth with a block to check the program and a separate application for adults. Please start using these
applications from this day forward.

The youth form is universal for all youth programs.

The New Look
The background of the new form is white, which makes it better for scanning On the form are blue boxes-one for each number or
letter entered. Items that are neatly printed and contained in these boxes will ensure a good quality scan.

Health History
Health History no longer appears on the back of the youth form. Unit leaders should use the standard Class I Personal Health &
Medical History form to obtain a member's medical history, parental permission, and medical release information.

Parental Signature for Venturers
On the youth form, a parental signature is required for all minors participating in Scouting programs. Venturers who are at least
18 years old can sign their own applications.

Best Practices for Completing the Form
For the best scanning results, pass along these tips for completing the form:
- Don’t alter the application in any way; it could affect the quality of the scan.
- Use a ballpoint pen-no felt-tip or gel pens.
- Use black or dark blue ink.
- Use upper case letters and stay within the blue boxes.
- Press firmly and print one letter/number within each blue box.
- Fill in circles; check marks may not scan.
